- MBBS with minimum 1st Div or equivalent from PMDC recognized Pakistani/Foreign medical college/university
- Completed minimum one year house job in institution recognized by PMDC
- Preference given to those with relevant field experience after house job
- Note: Candidates having cleared FCPS-I after Dec 25 are eligible to apply

- Intelligence Test: Followed by Academic Test in relevant subject at respective I & SC
- Initial Medical Examination: Physical Test and Interview at PAF I & SC
- Psychological Test: Of shortlisted candidates only
- Interview: By AHC Selection Board, Rawalpindi/Lahore/Narowal
- Final Medical: Final medical examination at CMH, Lahore
- Final Interview: By Final AHQ Selection Board, Islamabad
- Final Selection: By AHQ in order of merit

- Permanently declared UNFIT by Appeal Medical Board
- Declared medically unfit due to Hepatitis B & C
- Withdrawn from any Armed Forces college on disciplinary grounds
- Thrice screened out by Army/Navy/Air Force during preliminary selection
- Tested within 4 months gap of previous appearance in ISSB without waiver
